Can anybody help with how to intstall Diego's patch?

Unrar file and import into DT03.img

I dont know how to do this? When I try to extract to this file it says I cant do it.

Why is it that when I go to Edit League/Cup I can see the FA Cup and Konami Cup (as well as others) but when I try to play in a Cup the FA Cup is unavailable?
