It's not too hard. All of the things you want to do are done in the Tournament section. After you have created your leagues, you need a tournament for each which is basically just the league season. After you have each league defined click on the Team # (TEAM 1 , 2, 3 etc.) button in the third to last spot in the League A and use the drop down list to point to League B and select the team from league B that will be promoted to that spot when the season ends (i.e. Winner, 2nd, 3rd or whatever you choose). Do the same for the next promotion spot, then go to League B tournament and do the same for the teams being relegated from League A. Looking at leagues already defined like the Barclay's premier league will help you understand.

For the last item you'll need to create a separate tournament for just the playoff game. Make sure it takes place after the leagues finish play. Assign the teams using the Team button as above. After the tournament is created, go back to League A and B and assign the winner of the playoff to a spot in League A and the loser to a spot in League B. Good luck!

btw, is it possible to have it in career mode, I think I did it yesterday but my fifa started to debug or so, but I did alot more stuffs too so i dun rly know ^^

Yes, there is also a check box on the tournament screen for Career mode.

it's so hard lol

it always debugs when i play fifa in that league... :/
