Ric (or anyone)- Still have V3 3000 probs (__4X__)

hey Ric, thanks for the 3D setup files, and thanks to everyone else who took the time to send patches, fixes and suggestions (about 6 of you...)
However, I tried all of the things suggested to me, used all the files sent, and tweaked what people told me to tweak, but to no avail... i still have the same problem
Here it is in case you haven't seen it:

I Swapped my Diamond Monster Voodoo1(4M and ATI Rage Pro AGP 2X cards for a brand new 3dfx Voodoo3 3000 card. I removed all drivers from my other cards, and have the latest set of 3dfx drivers released late July. When I try to run 3dsetup.exe in my FIFA99\setup directory, my screen goes blank and the hard disk whirrs and stalls. Then my monitor sounds as if it is turning off and the whole computer crashes. I can't even re-boot using Ctrl-Alt-Delete. This forces me to play FIFA 99 in software mode as it can't detect my superb new V3 3000 and doesn't remember my old settings from the V1.
I'm running the patched version of FIFA 99
Does anyone have any ideas? Should I contact EA for tech support (it's the only game I have probs with, the others work fine...) or should I go to 3dfx (even though it's only this one game that gives me hassle) for help?

As far as I can see I have absolutely no driver conflicts from my old and new software, and no other hardware conflicts on my computer (this info from Windows Control Panel). In case you need it for any advice I am running the following system:
PII 350, 64MB RAM, 3dfx V3 3000, TV Tuner card, SB AWE 64 Value, DVD, 56K modem, and a network card.

Try removing your tv tuner card, I know it can create conflicts sometimes.
