You put the .dds file in your FIFA 16 directory.

FIFA 16>data>ui>imgassets>heads

Then you regenerate your files

Do you know how I can get rid of these pixelated holes on the shirt? I notice that these pixels only appear after importing it with CM16 but i m not entirely sure if thats CM's fault.

worked fine for me(imported using cm), maybe try changing the size of image to 128x128.
anyways here is the pic.

use i68 controller for regeneration, fhl-bh editor doesn't regenerate minifaces. or you can manually import the png using creation master 16.
