Yes, they do work. In AMD set Enhanced v-sync and instead of Global, select Game profile and cap frames there (sorry forgot the name of the setting here, but it's easy to find, just set it to on and cap at 60). Voila. Super smooth gameplay and menus/cutscenes flowing at 60fps ;).

Played 22 games have seen it only once. They are just not implemented like they were last year (that's actually good as back then it happened WAY too often). ;)

Sadly, that's something Fidel can't do either. As you said, it's all in source code, so whole defensive mechanics CAN only be done right by EA, if they are willing to do that. I find that attack is heavily in favor this year over defense. Heck keeping clean sheet is a task on its own...

I am playing on vanilla game, without slider edits yet. I enjoy what I see (fouls, cards, etc.) and the game really shines this year. Of course there are niggles where I feel it should be better, but will leave that part to EA. Hopefully they will patch it. Plus, it's quite a shame regularcat...
