I noticed that for the 86, 90 and 94 world cups the fixture follows what actually happened rather than get the best 4 third placed teams to qualify.

Here is a template of the 1986 WC allowing the best 4 thirds to qualify to the round of 16 rather than the third placed of the groups that actually made it. You can copy this template for the 1990 and 1994 WCs.

While EA did a really good job on Rudi Voller, Jean Papin or Marchisio, players like Toure Yaya looks worse imo and Mascherano only got hair update ))
I will also send EA version of Gerd Muller to Acicog although we already got a better version(the one with beard)
for the rest of the new icons&heroes we still have to wait unfortunately

How do I do that ?

In CM16 click on the cup (Tournaments tab) then click on the country (1986) then click on the downward red arrow (load competition patch) then adjust graphics and names

You can also edit the tournaments directly on settings.txt in the dlc folder. Just add two setup stages after the group stage. The first one with the six third placed teams (check keep standings) and the next one with the 4 best teams (check sort by competition average). Later on the Round of 16 you put the winners of groups 1 to 4 with "a" team of the second setup stage checking Random draw event and avoid same group.

That way you have the tournaments set up correctly. In the case of WC 1986 for example, you could qualify to the Round of 16 as 3rd placed team of group C or D, provided they are among the four best ranked.

There is also a way to setup knock out legs to be replayed in case of a draw, as it was the case from 1930 till 1958 (it happened in 1938 between Cuba and Romania or in 1934 between Italy and Spain, for example) and as it was for the final until 1982. The only setback is that the first leg does not go to extra time, it finishes at 90 minutes and then only the replay goes to ET and penalties.

I couldnt figure out a way yet to make ET to work for the first leg if you set up a Replay
