this problem happens when converting a face from fo4/switch or others to fifa 14 then to fifa 16 with the last version of tokke's fifa converter.
to fix it you need to convert original files directly to fifa 16.

Every home game for Benfica and Porto crash for some reason, unless I manually change the stadium, which I can't do in tournament mode.

I changed their home stadium in CM16 to El monumento and now it works. I don't know why. Also I can't Ibrox to work with the stadium server in tournament mode, it only registers in friendlies.

guys, to avoid stadium server problems i suggest you to add stadiums manually, like adding the files to StadiumGBD folder then writting the lines in settings.txt file from FSW folder.
for me this works the best without green and blue squares textures and so on...
although stadium crashing might be a problem regarding glares... i dont know

Mhmmj this sounds very complicated
I hope we won’t have the stadium server in fifa 14 because there are lot of problems

in 16 the stadium server is only there because of the limits of the game
ACICOG explained the inconvenients some weeks ago and explained the reasons

But generally the CP 16 is not as stable as the 14 especially because of stadium crashes

The problem is in 14 the Stadiums are directly imported in the DB
Without Stadium Server
When you make this for 16 you are not able to play night matches for example
