There can be other ways to solve face crash in game, but here is how I made it work:
Before following the steps, please make sure you have the following 3 downloaded:
1. SetFaceType (credit to scouser09)
2. DB Master
3. CM 16
Download links are all provided in my previous comment.

Steps:
1. Put "SetFaceType.exe" and "SetFaceType Run FIFA X.bat" in "data\sceneassets\" folder.
2. Extract players table with DB Master and put in "data\sceneassets\" folder:
2a) In DB Master, File -> Open DB
2b) Choose "fifa_ng_db-meta" and then "fifa_ng_db" (both in db folder of fifa game)
2c) Choose "players"
2d) Table -> Export Single, we get "players.txt"
2e) Put the "players.txt" in "data\sceneassets\" folder (in fifa game directory)
*These is no need to close DB Master after this, we still need it afterwards
3. Run "SetFaceType Run FIFA X.bat" in sceneassets folder to update "players.txt" information
*If a real head exists, headclasscode will be set to 0 and hashighqualityhead will be set to 1. This will avoid the need to do this manually in CM16. - By scouser09
4. Reimport players table with DB Master and save. Again in DB Master:
4a) Choose players in the list
4b) Table -> Import Single -> Select the "players.txt" in sceneassets folder we just updated in step 3
Now here is where I got confused since I still had crash in game. I finally got the solution yet which is step 5:
5. Save it in CM 16, in CM 16, select:
5a) File -> Open-Fifa 16
5b) File -> Save

I have no idea if there is anyone who got it right even without step 5. However, please note that original saved data in career mode will be destroyed after saving with CM 16. You may start the game after first 4 steps if you do not want to lose you career mode data. If it does not work, go on to step 5.

I can't performing step 5 that why my game is crashing.

You don't need to save with CM16, just save it with DBM and it's done.

Yeah. From those steps u didn't save the database in DBM so your database didn't make any change, so that s why your game crash.

Sorry guys, I had to buy a new computer and I lost most of the graphic mods I had. Can some please share:

1) Flags folder.
2) Corner flags folder
3) 20-21 Scoreboards folder for CG server.
4) 20-21 Movies folder for CG server.
5) 20-21 TV Logos for CG server
6) 20-21 Stadiums for SG server
7) 20-21 Banners
8) Trophy models

Any help is really appreciated.
