Need Help Importing Hair/Head

In FIFA 12 this worked fine for me, but now in FIFA 13 when I try to change a player's head/hair their hair is glitched out, like this:



It looks that way in game...

How do I fix this?

Could someone at least list the steps in changing hair because maybe I am not doing something right. I've seen numerous videos.
