After some time I have managed to add arsenal third kits using this method.(well slightly modified)
When adding the third kit for any team (I use Arsenal as reference) it should NOT show up as an image in the creation centre, instead you should see the left hand data fields filled as per your editing of the database .mdb file.
To fix this you should open up zdata04.big with bigGui and navigate to the file you created(should be playerkittexobj.texobj463104248046875__texture23__3_0_1), extract this file to your fifa root directory (normally CProgram Files\EA SPORTS\FIFA 2004),then delete this file from your zdata04.big.Rebuild the archive.
In creation centre you will then need to set the font colour and collar geometery of the kits as they will be wrong.(You should not see the kit image within CC now.)
Also the method for adding minikits in the tutorial did not work for me what i had to do was open up the minikits directory and copy any team with third kits big file( i used k2.big) copy the file and rename it to the team id you want(e.g Arsenal is k1.big).
Open the file using bigGUI and rename the files contained within it to you teams ID number.
Next export the FSH file from the archive to desktop and open with FSHTool,Replace the three bitmap images(0000=Home,0001= away and 0002=Third)with the minikits for your team.
Now delete the FSH file you extracted to the desktop and create a new one with FSHtool by double clicking on the index.fsh file.
Now using bigGUI you can import the newly created fsh file from your desktop back into the k1.big file.DO NOT rebuild the archive.( I found that if the offset values changed at all during this procedure the minikits would not work in the game)
I 'm sure someone will come up with an easier method but this way worked for me.
