No, Real didn't work for me either. Real home and Juve home are kits you simply can't replace with CM, it just won't work, but don't ask me why.

This is how I did it: I searched in CM for a random white jersey with black or dark blue numbers. I cloned it to Real's 4th kit spot. Then I overwrote it with the Real kit & badge.
So now I have the old Real kit as home kit, and the new one as 4th kit. So you can always choose the 4th whenever you want.
It's the only solution I could come up with...

please tell me how to use your pics? your posted pics is 150x150. it can't match in CM12. When I resize to 1024x1024, it can't match as well.
