Rx3 file are imported in game as they are without any manipulation by CM, so very likely, the problem is in the rx3 file you are using. About formation: I don't think we can remove it but we can use generic formations for additional teams. CM 14 already allows this.

Open the rx3 texture file with Rx3Master, extract the textures as images and import them in CM14, now the Rx3 is created by CM14 and it will work.

5- Regenerated files (you can do with i68Regenerator, the CM14 itself, and other programs)
Now the game wont crash, and you have the custom faces instead of generic ones. If you don't see the changes, try to delete "Squads*" file from Documents/Fifa14 folder.
