Vertical sync should always be set to "always on" unless you like tearing and vertical lines on the screen. Also with PES you should force the monitor refresh to 60Hz. Best way to do that is with ATI Tray Tools. What I do for my ATI card (9800 Pro) is just install the drivers from ATI's site without the catalyst control panel which is crap and forces you to install .NET which is yet still more uneeded crap. Instead, just install the driver alone and then you can use ATI Tray Tools to change any driver settings along with having a bunch of tweaks and features like game profiles that the ATI control panel doesn't have.
Anyways, I don't play the PC version anymore. I have a PS2->USB adapter and the game just doesn't play right on it. Didn't really play right on PES5 either. PES4 was perfect though at that time I was using PS1 controllers. Busted those old controllers though and now I can't find anymore in any of the shops near me. Tried the Logitech Rumblepad controllers but the d-pads on those are horrible in comparison to the PS2 controllers so enough of that. Imported the PS2 version of PES6 to play on my modded PS2 and its fricken perfect. The game is fun again. Konami doesn't give a **** about PC users, never going back! Can't wait for the PS3 version which should be the true next-gen version.
